• You may need to change that you are paying for an item or service to “FRIENDS AND FAMILY” - otherwise we are charged a fee per transaction.
• Please use one transaction to pay for all players
Winter/Indoor and Spring/Outdoor Season = $225/child. Multi-Child discount ($25/child) for each registration beyond the first. (i.e. two children = $425 total)
CIPPE Forms

CIPPE forms (and the required physical) are required for any PIAA sanctioned sport and 7/8 grade soccer will be PIAA-sanctioned for Spring 2025.
If you are choosing to have your family physician (or other health care professional outside of the school) complete the physical, there is nothing that needs to be done at this time. CIPPE forms are due before we begin practicing in the Spring (end of March) – these can be turned into Mrs. Bollinger in the Junior High office. If you wish to have a physical performed through the school (at a cost of $15), these will be occurring sometime late January.

CIPPE forms for Fall 2024 or Winter 2025 student athletes. If you are a 7/8 grade boy/girl who participated in a Fall 2024 or Winter 2025 sport and already completed a CIPPE packet, you simply need to complete SECTION 8 (recertification by Parent/Guardian) as long as there were no injuries during the season or changes in your health history. If there are any changes to your health history or injuries, then SECTION 9 must also be completed (re-certification by physician). The form indicates, “this form must be completed not earlier than six weeks prior to the first practice day of the sport.” As such, I might wait until about mid-February before completing these if you fall into this category.
